The category is determined by the assessment evidence — you don't choose it, the assessor recommends it.

SDA funding is paid directly to the property owner. You pay a "reasonable rent contribution" — typically 25% of the Disability Support Pension plus 100% of Commonwealth Rent Assistance — from your personal income. SDA does not include support workers; that's funded separately as Supported Independent Living (SIL).
