2. “A condition has been imposed, for providers of 0107 Assistance with daily personal activities, for risk assessment of participants who choose to be supported by a single worker.”
3. “A variation condition is imposed for the assessment of Module 5A: Assistance with SIL. The audit must be conducted as part of the provider's mid-term audit which must commence no later than 18mths after the date of registration.”

What to ask before choosing an NDIS provider
Ask which of your supports they can actually deliver in your area, and how soon. A broad registration means a provider is permitted to deliver a support — it does not tell you whether they have capacity for it in your area this month.
Ask who your regular support worker or nurse would be, what happens when that person is unavailable, and how the organisation handles complaints. For a registered provider, complaints can also go to the NDIS Quality and Safeguards Commission.
Ask what their cancellation policy is and how much notice they need, and how they charge against the NDIS Pricing Schedule — particularly for weekend, evening and public-holiday rates.
